  • First reqs gathering session - go through data fields and confirm what data points are required, etc.

  • Get contacts (names & emails) for the HA submitters and reviewers.

General Notes

  • We need to see what fields could be pre-population

  • Maybe we can make things easier/more efficient with one simple report.

  • The Full Income Statement (FIS) is an ideal state (we will need to expand on this in further mtgs with Bonnie & HAs)

  • The whole compensation piece is a challenge. Need to be able to break out direct care and non-direct care portions. VIHA (and likely other RHAs) need to be able to pull it out.

Spreadsheet (web tool data collection) Notes

  • Facility info & other info - these sections could potentially be pre-populated.

    • Site ID = locked field. Bonnie could probably pre-populate from DSR. Important for HLTH but not as important for HAs.

    • new field → May need to add contract ID (FHA) - not mandatory. 

    • Program type = this may have to be expanded to include Assisted Living (AL). Display but it will be pre-populated.

    • Ownership type = Dropdown menu that lists NFP or FP, O&O, Owned & Leased (only will be a handful of facilities). Domain of values.

    • The info in the accreditation section will only change once every 3 years (pre-populated from previous submission).

    • Local Health Area (LHA) --> there may be a way to tie the postal code and the LHA

Owner Info

  • the Contact section (including tabs below will not have that level of detail).

    • Operator Information

      • Owner and operator could potentially be different but most of the time the two will be the same. Add checkbox that says "if x…".

      • Owner could be a corporate site itself or a home address.

        • What level of detail do we want to include? There can be a sub-contractors sometimes. Because of this, we may want to add a note on the staffing level.

Preparer Contact Info (contact person listed for if HAs have any questions) 

  • CFO or VP finance contact info

  • Auditor's contact info. Can be pre-populated but may change every 2 to 3 years.

Tab #2: Bed Inventory & Occupancy

  • Could potentially pre-populate lots of this info from the HCC bed inventory that is filled out by HAs.

  • Will need to identify scope for 3.36 reporting

Private beds section

  • It is possible for the location for the beds to move around, so at some point they may be entirely co-located and at some point, they may be in an entirely separate wing/unit. (For the sites that have two respite beds, we have to figure out how to distinguish between those).

    • A subtotal for publicly funded beds to be displayed and a subtotal for private beds to be calculated.

Bonnie to send CGI the list of HA contacts to schedule 1:1 UX/UI interviews with.
