It appears that the permissions for this feature are limiting what a user can see:
The highlight section below doesn’t show up if the form submitter doesn’t have specific roles granted to them on the team management page.
I believe the correct behavior should allow anyone who can submit a form to use the uploaded template that the form designer uploaded in the form settings.
