Form columns missing from selection after being de-selected

Description:

Default columns (Submission Date, Submitter, Status) are no longer available for selection if they are de-selected, and I click on one of the other form columns.

This is a bit of a tricky one to reproduce.

Select 1 or more form columns, de-select the default columns. Hit Save.

Open

Re-open the select columns (the default columns should still be displayed), and uncheck one of the fields. Upon clicking the default columns disappear permanently for the user, and there does not appear to be a way to get them back without creating a completely new instance of the form.
